I'm not a medical doctor, but I do know this much about creatine and kidneys. Creatine metabolizes to form creatinine, a waste product that is excreted by the kidneys. The kidneys can only rid your blood of so much creatinine (the waste product) per time period. Taking creatine monohydrate may result in a higher serum (blood) creatinine level since the kidneys cannot rid the blood of that excess creatinine fast enough. It may also force the kidneys to work harder in trying to rid that excess creatinine.
Remember, creatine metabolizes to form the waste product creatinine. You should discuss this in detail with a medical doctor, preferably a nephrologist. Any advice from weight lifters, personal trainers, or companies is bound to be biased. They do not have experience dealing with kidneys, livers, or other organs. They only know what they see in the gym.
